This form can be used for all programs except trips.
How to Register:
Once you have downloaded the form you can do the following to get it to us:
- Fax the registration form to (757) 868-3585 or
2.Drop off the registration form during business hours (Walk-in) or use our after hours drop box on the front of the Municipal Building at 830 Poquoson Avenue. or
3.Mail the registration form to:

Please note that on all credit card transactions for payment by order of the Treasurer, a 2.5% fee will be charged for the use of the card to all transactions.
If you are unable to make it to our office, please utilize the drop box on the front of our buildings for REGISTRATION. The Drop Box is located at 830 Poquoson Avenue, Poquoson VA. Please put forms and payment in an envelop within the drop box afterhours if necessary.
